But when it comes to his personal life? He’s been surprisingly low-key, especially about the woman who’s been by his side for more than a decade. In case you didn’t know, the actor has been with Heather Milligan for quite some time now.

The “Terminator” actor met Milligan in 2012, right after his very public split from Maria Shriver. It was just before filming “Escape Plan” with Sylvester Stallone. Recovering from shoulder surgery, he wasn’t looking for love; all he wanted was a physical therapist who could whip him back into fighting shape.

His surgeon didn’t hesitate, though, and knew exactly what he needed. “There’s only one [person], it’s Heather,” the actor told People. “Any athlete, I always send them to her, because she’s the only one that really knows what they’re talking about.”

And so, in walked Milligan, a former competitive gymnast turned physical therapy powerhouse, with a resume that reads like the dream team of sports medicine. The two worked together during his recovery. But surprisingly enough, sparks didn’t fly immediately.

“I called her back after therapy was over and took her out to lunch to say thank you,” he said. “And then one thing led to the next.”

Who Is Heather Milligan?

If you’re imagining a Hollywood starlet clinging to Schwarzenegger’s arm, think again. She is all about work ethic and a peaceful lifestyle. Still, her resume is pretty impressive, if you ask us. She holds a doctorate in physical therapy, an MBA from the University of Nevada, Las Vegas, and more than two decades of experience treating top-tier athletes. Yes, everything from UFC fighters to NFL pros, wild indeed. She also runs Elite OrthoSport, one of L.A.’s leading sports therapy clinics, and she’s so respected that Schwarzenegger’s surgeon basically said, “Don’t even think about anyone else.”

But Heather’s no stranger to competition herself. She spent 20 years as a competitive gymnast, which probably explains how she keeps up with the man who still bikes around Brentwood like he’s training for Mr. Olympia.

She’s also deeply loyal to her family. Schwarzenegger has praised her devotion, revealing she cared for both of her parents when they were diagnosed with cancer and still calls her grandmother every single night. “That’s when you really get to know a person,” the actor said. “The heart they have.”

A Decade of Keeping It Chill (& Cute)

The couple’s been going strong for over ten years, quietly showing up at events here and there, but they keep it relatively private. So, don’t expect constant Instagram PDA. Neither of them overshare, and that’s kind of the charm, really.

Still, Milligan makes occasional appearances at red carpets, or big political or movie events. She also seems tight with Arnold’s five kids (Katherine, Christina, Patrick, Christopher, and Joseph) and has been spotted dining with the fam, including a 2018 brunch with daughter Katherine and her now-husband, Chris Pratt.

“Chris and Katherine met up with Arnold and his girlfriend Heather for Sunday breakfast,” a source told People. “Arnold seemed happy they joined him.”

That’s putting it lightly; it’s safe to say the actor is clearly smitten. In a 2023 interview, he gushed, “We have so many things in common. I think the world of her. She’s clearly independent. She just is driven like hell.”

Football, Donkeys, & Domestic Bliss

What we really know about the mysterious but not-so-mysterious woman is that she’s a huge sports fanatic, especially football! And she’s converted Schwarzenegger into a semi-fan too. “She sits with me and explains everything,” he said on Travis and Jason Kelce’s “New Heights” podcast. And let’s not forget her love for animals. Her Instagram is filled with snaps of Schwarzenegger’s famous mini pony, Whiskey, and donkey, Lulu.

While Arnold’s been married before, this new chapter feels different. It’s grounded. Unflashy and well-earned.

And if you’re wondering whether the ex-bodybuilder is still romantic after all these years? He sure sounds like it. “Ten years ago, I fell in love with Heather,” he told the Kelces. “I love that she’s into working. I love her independence. And we just have so much fun together.”

So, she may not be a movie star, but Milligan is every bit the powerhouse. Smart, athletic, grounded, and fiercely loyal. Because, behind every legendary action hero is someone just as tough, only with a therapy table instead of a bazooka.
